This guide serves to provide a relatively simple procedure as to exactly how one should establish their own dedicated server to allow more than four human players in Campaign, Left 4 Dead 2. Go to Steam -> steamapps -> common -> Left 4 Dead 2 and delete Left 4 Dead 2 folder after uninstalling it through steam. After it's uninstalled from steam, right click and go to properties tab -> Local Files -> and verify integrity of files. This will reinstall the full game. Look in your left 4 dead 2/bin folder. Towards the bottom there should be the VPK.exe. Don't move it from that folder, but create a shortcut to it. Place that on your desktop or something, and when you have your finished sound folder, drag it onto the VPK.exe shortcut. The program will do the packing.
